Implementation of Public Health COVID-19 Passenger Self-Declaration Form / Passenger Locator Form

We have been informed by the Ministry of Health and Wellness (MOHW) that the following forms have been uploaded on their website at health.govmu.org :

  1. Public Health Covid-19 Passenger Health Self-Declaration form
  2. The Public Health Passenger Locator form

With immediate effect, passengers travelling to Mauritius (international flights only) are required to download and fill in the forms prior to check-in at the departing country.

Please note that upon arrival, the passengers should hand over the two forms together with the Health Declaration form to the MOHW.
